72 yrs old male, cancer free for 4 years, other meds for blood pressure and cholesterol with good h/w.

Mainly a simplified issue guy so i'm not sure how would he be rated between companies.

Currently has a term to age 100 with $600 month thats increasing significantly each year
 
You need to find out the following in order to obtain accurate quotes for him:

1. Date of diagnosis
2. Cancer Stage - T1a, T1b,T1c, T2a, T2b, T2c
3. Gleason Score
4. Pre-treatment PSA level
5. How was prostate cancer treated - surgery, radiation, etc?
6. When was treatment completed?
7. Post treatment PSA level?
