2013 CMS Generic SOA Form

Thanks for posting the form!

But can someone offer a reasonable explanation on why CMS wants non-Medicare plans--of which they have no control--on the Scope now? :goofy:

Because before we weren't supposed to offer the Hospital Indemnity plan along with MAPD's and I was told now we can do them at the same appointment if the SOA is filled out as such. Those 2 plans kind of go hand in hand so it's nice they finally have this. They do have control as to what you are talking about combined with a MAPD or PDP. Such as you shouldn't be talking about Life Insurance or LTC Insurance at the same appointment which you probably already know.
