I assume that not many companies can do HBP+Insulin Diabetes for 36 year old male. He is too young for some companies. Any suggestions?
 
What's his A1C and glucose readings? Is the blood pressure untreated or under control with medication? 36 shouldn't be too young....
 
well controlled juvinal onset diabetics start out at table 6. Depending on height/weight, history of control of both DM and BP a table 4 could be possible.
Does he know what his A1C is?
 
Table 6 with Genworth would be $23.84 50k 20yr term... but fully underwritten
West Coast t-6 100k 20yr would be $34... but again... fully underwritten.
 
easiest option is UHL... I don't know if it's the "best" option. If he can qualify with underwriting... he saves $5,000 over 20 years with the 50k genworth. And table 4 is not out of the question...

Does UHL issue if there has been an MIB hit (decline)?
If they do... I would put the UHL infiorce on a monthly basis then apply with Gen or WC and see if I couldn't cut the premium in half.
