Applying for State of Texas Life&Health Agency License

Apologies in advance if there is already a thread that explains this, but I attempted to search before I wrote this post, and couldn't locate any info.

We are ready to file for our State of Texas agency license for general lines, and I've run across two forms that appear like may be the right ones. Hoping someone can assist. I found the forms 205 and 206.

I'm not exactly sure what the difference is, but am I at least on the right track? Will one of these be what our principal (owner) needs to fill out in order to have our LLC licensed with the state?
