Change of agent letters for United Health Care

is there a form to use for "change of agent" with United Health Care?

UHC does not honor AOR changes on Med Supp. They might on MA, I'm not sure as I don't write MA.

They don't allow AOR changes on Advantage plans either.

But you can write the a different UHC plan if the previous agent failed to put them on the correct one....
