Changing MAPD/PDP plans mid year

Never gave it much thought before, but for someone, who for example is on Medicaid, or in ALF/NH, or moves out of the service area, and can then switch to a new MA/PDPplan outside of AEP, what happens to whatever deductibles/donut hole or MOOP's that may have been satisfied under the original plan for both Rx and A/B charges? Let's say someone moves from Humana MAPD to Aetna MAPD....for whatever reason.....is there any coordination between carriers that would make sure the person is not starting from scratch on Rx or MA MOOPS again? I would assume there is a mechanism, but not sure how it would all work....or is that info always posted to a centralized Medicare data base as well as the carrier....or maybe there is a problem if you change

I know if you change Med Sups in the course of the year, Medicare keeps track of Part A/B expenses.
