Possibly Great Western. Several have a true Graded, like Aetna and Bankers Fidelity. While not Level, Graded does provide day one coverage.

It does say 2 years, but all the CHF meds say modified on the Rx guide. It might go through, but I doubt they will pay.

Prosperity doesn’t do ‘risk assessments’ when I called to ask, nor does it seem that appitical is the final say. Did a case with them, a week later got an email that they were calling the Person for more info.
