Devin, yes this may be written in NJ. I picked up this contract recently as I ran into a referral who could have used it (he's in MD). But he's Middle Eastern and feels he doesn't need it, due to his non-western diet and lifestyle. His much-younger wife and kids are on a non-ACA compliant plan.
On piu.org, there's a page called Forms and Brochures. Six States, including NJ have affidavits. One may need to be completed for this product but I'm not sure.
