Embedding Fillable PDF on Website?

Always looking to test out new things, I'm wondering if its possible to create a PDF Quote Sheet, that would be fillable, and embedding it on to a website? That way a visitor could fill it out, press submit, and the pdf form would be emailed to our agency?

I dont think ive seen this done before, and i realize it would rely on someone having adobe reader installed on thier computer to work, but thought it may be a possibility

Why do this when you can have a form capture the same info and email it to you and you won't have to worry abouthaving adobe reader.
 
You can't make a pdf look nicer than html, and all you're doing is adding unnecessary overhead to the website by doing that.

Rather than embed a pdf, find a web designer that can meet your criteria for the html form.
 
No matter how you end up doing the pdf, they would still have to upload it back to you some how.
 
I think, now don't hold me to this, but I think you can make a fillable pdf in acrobat that you can embed into the page, and can have submitted fields, that will send data back to a website.

I've never done it, I just know a couple insurance companies do their apps that way.

I think with a pdf you might overwhelm with too much of a form. Try something where you can have multiple page forms so it's not so much for them as they fill it out. I've got a great example in my signature. It's not so much what you ask but HOW you ask it.
