If an employer got a signed waiver from an employee at Open Enrollment and the employee changes their mind 3 months down the road, assuming the carrier accepts a retro-enrollment, does the employer have to contribute their portion of the back premiums?

Can they pass the entire premium on to the employee for those 3 months?

My understanding is the employer would have to pay. Retroactive wouldn't change the employer contribution obligation. A carrier rep should be able to confirm.
