Have you ever had to 'Fire' a client?

Absolutely, yes I have.

The key is to identify certain flags and markers before they become a client, and before they bleed you of your time and energy. Often during the quoting process, I give them many opportunities to fire themselves.

Off the top of my head, some of the identifying markers are, bad driving history, bad credit, lying, old agent not wanting to work with them, a high number of claims, calling multiple brokers for quotes, debating and arguing, gaps in prior coverage. None of these items are guaranties of a bad client, but definitely are signs.
