If you are looking for something for folks under 65, it is hard to beat Manhattan Assurance (CUL).

yes, yes, yes if you are set up for advance. They give you a link (upon your request) for online apps... Here is mine. The esignature is the clients mother's maiden name.

Check out SureBridge. They just launched a new Hospital plan with an option lump sum rider and a bunch of other riders. True Simplified Issue (no PHI, MIB, RX Check), issued from 0-90, strong comp.
