Indexed Annuity 401k

Does anyone know of a company (other than ANICO) that has indexed annuities that can be used in an employer's 401k?

I did a round of calls looking for someone to take a cash balance plan and several carriers were open to having the indexed product in the 401k (although I didn't have much luck on the pension side which is where the agent wanted to place it).

Off the top of my head, National Integrity and Great American both seemed open but again, it was sometime last year so I may be misremembering.

You could start with those two.
