I have replaced this crap 100 times. If you call to cancel, they put you on a line that is never answered. You have to email and fax a letter in to stop the insanity. They're based in Boca Raton with the most laughable Board of Directors I have ever seen.

Sounds good, but few claims ever go over $2M. If so, they are usually approved for Medicare by that point.
