ACUSA Membership Plan . It says that "The plan is provided through a membership in the Alliance for Consumers USA (ACUSA) which includes Limited Medical Insurance Coverage underwritten by AXIS Insurance Company, as well as several other savings benefits and programs".
How good is this Health Insurance policy?
Does it full-fill the mandatory requirement of Health Insurance to avoid Penalty while filing tax return?
States it does not provide major medical or comprehensive medical coverage and not designed to replace major medical insurance.
Heard customer service is bad and getting claims paid is hard. Any comments?

It's not health insurance and not compliant with the Affordable Care Act, other than that you are on your own.
 
If you are not familiar with Indemnity or Limited Benefit Plans they pay a fixed dollar amount per procedure or inpatient stay. Without seeing the plan it's difficult to comment on the benefits. However, there are a number of these "Association" plans out there and if it is like most, it is probably a very limited benefit plan. A lot of these plans will have a base indemnity plan and try to load the prospective client up with ancillary products like CI, Acc, etc. Plus, they will charge the prospective client monthly association dues. If the plan is explained properly and the client knows what they are buying that is one thing, however a lot of agents out there sell these plans as 100%, no deductible plans so the prospective client thinks they are buying something completely different then the policy they are actually getting.

If these policies are purchased as stand alone policies, they do not meet the mandatory requirements. So unless they are using this policy as a supplement to an approved and compliant policy they could face tax penalty consequences.
