I also can't speak for the other states, but it seems that benefit with Anthem is very strong in VA compared to the rest of the market given that there is no additional deductible or copay on top of the plan's overall deductible. Doesn't really make sense for a younger woman not to buy it at that price.

Anthem is the only maternity player in VA that matters. GR does not offer maternity in VA.
The $4000 benefit was eliminated on GR maternity here in Texas the max is $2500 for year 1 and year 2, pays at 80% - so short answer max $2000 payout years 1 and 2, beyond that is not worth discussing since they won't be on the books anyhow.
