Medicine Exception Coventry

Does anyone have the number the provider (doctor) uses to call Coventry to get an exception for a medication?

Or, is there a form to complete to turn in to coventry?

This is for the state of Georgia, for Coventry Medicare Advantage.

I would think that you can find that on Coventry Medicare: Advantage & Medicare Part D Plans. There should be a phone number for drug related issues for your geography. I'm in Kansas City and I believe the Dr. office can just send the request. I don't believe we have a specific form here. Most of the doctors already know how to do this, but, I'd search the website or call broker services.

On a side note, Coventry approved Ciallis for an off-label use, albeit at the Tier 4 copay. So, it actually does work.
