Hi,

Can anyone provide information on what is needed when filing contractor application?,

For example
Geotype
Loss runs
Budgets
Etc....

And how to fill out acord 125,126, 140 for contractors.

OK - I'll try it this time:

You need an accurate description of the operations, property values, and GL class code with payroll. Also, you will need prior carrier info, loss runs, and possibly a Supplemental application to provide the additional details that many carriers ask on contractor.
