Need Help Getting Out of Network Benefits (United Healthcare Choice Plus)

So i am seeing someone for pelvic pain massage physical therapy and they are out of network. it is a very specialized field and not many PTs do it. I called and UHC gave me the entire list of pts within a 30 mile radius (a prerequisite for going out of network is that no one within 30 miles of my home can do this) i called in to get a geographical gap and they asked me to have my GP give them the codes they use for billing as well as the pt's tax id... The codes for billing are general cpt codes that are used for any physical therapy. they went off those codes to 'find a provider' and sure enough they do not do what i need to be done...

I then was instructed to file for a clinical geographical gap (i think thats what its called) and i was denied again. they have NO ONE in network. how do i get this covered?
