North American Life Insurance Co Partial Surrender Form

Okay its Friday night and North American is closed. I do more of my business with Midland and locked myself out of North American's Website. Anyone have a partial withdrawal form, which I believe is form 6773Z and is there a way to state a gross deduction amount as opposed to a net deduction amount ie client wants to withdraw $2000 from account and recieve check for the $2000 less surrender charges and withholding.
