They're responsible for a LOT more than that.

As a practical matter, they'll have to pay for the entire out-of-country service out of pocket - and file a claim (out of network) when they get back to the U.S.

It's a surplus lines product. If you sell enough of it (and I don't) you might want to pay extra for surplus lines coverage, but I don't see it being an issue.
