Personal Privacy Information: CMS

One take away from the 2015 CMS training for agents/brokers which was included in the 2014 training was the requirement for agents/brokers to have and use a document to use with clients that discloses their privacy rights and the agents/brokers responsibilities to safeguard that information.

Does anyone have a copy of a document that they are using with clients that they would like to share with the group or individually?

Also systems online for storing the records for (10) years.

I use SOS Online Backup for storage. It's HIPAA compliant.
