Years ago I was an independent agent with Provident Mutual. In fact, they were the first company I licensed with after passing my insurance exams. Some years ago they were purchased by Nationwide Insurance so I don't think they have independent agents anymore. However, I am not 100% certain of that.
Nationwide demutualized Provident Mutual in 2002. You may wish to "google" them for research or contact the company directly.
From what I can see it does not look like you can get appointed with them. I called in to their 800 number, and was surprised. They really did not want to talk fixed annuities to me,wanted to puch vairiable HARD. I pressed them on a rate and they finally quoted one, then offered to do it all through the mail. I did not think you could do that?
