Yrs. In Practice: 6 yrs

Medical Conditions: Diagnosed with Acute Promyelocytic Lukemia in April of 2006. Recieved chemotherapy in 3 stages. Goes for blood draws twice a year. Doctors perceive a low instance of relapse. Chemotherapy ended in September of 2008.

Been Cancer free for 6 yrs.

Current Conditions: Goes to a chiropractor, on average, once a month for regular adjustments.

Current Medications: Takes generic for Emitrex for Migranes as needed

Try Union Central, Guardian, or ON.
 
Union Central is now Ameritas. I call them UC still out of habit.

My ON rep told me they were loosening up on UW some with Di... lol. But this is a hard to place case.

Try Illinois Mutual or Principle. If all else fails Petersen Int will take him but it wont be cheap.
