Yes to Risky Activities in Health Insurance

Anybody know what happens when you say "yes" to that question on a health insurance app that asks if you do risky activities like skydiving, rock climbing, etc?
 
What's "a little"? Like $10, $20, $30 a mo extra, or like $75, $100, $125 a mo extra? I know it's difficult to tell without specific information, but on your experience a best estimate?
 
They probably have to fill out a supplemental form, then get rated depending on the danger of their activity.
 
The better question is, what happens if you answer no but you really do participate in those activities.

Likely an exclusion to a decline. You'll have to answer additional questions - like frequency and in case of say, scuba diving, how far you dive. Does "rock climbing" means at your local park, or Mt Everest? It matters.
 
Isn't it pretty obvious that if you answer "no" you will NOT be covered by injuries from any of those activities?
