Waiver Granted by the U.S. Department of Health and Human Services
All companies selling individual coverage in New Jersey are required to offer the limited benefit
coverage, called the Basic and Essential health care services plan. Horizon Blue Cross Blue Shield
of New Jersey provides that plan as the Horizon Basic and Essential (B&E) Exclusive Provider
Organization (EPO) plan. It also offers an enhanced version of the B&E EPO plan called the
Horizon B&E EPO Plus plan.
The Federal Patient Protection and Affordable Care Act (PPACA) sets a limit on the minimum dollar
amount that can be applied to key benefits on an annual basis. However, PPACA allows the
Secretary of Health and Human Services to waive these annual limits under certain conditions.
