Coventry of kansas (and western mo) is $200 plus 3% for first year with 3% renewal. The $200 is per app, not per family member or individual, unfortunately.
Coventry of Missouri (eastern mo) is 5% all years.
Anthem blue cross blue shield is $17 per person per month first year, $12 per person per month renewals
Bcbs of kansas city is 6% all years, but premiums aren't really competitive
