Deductible - is this an annual amount? Meaning if my plan has a $1000 deductible and I have a $1500 visit on March where I pay my $1000 deductible and my plan pays the balance of $500 hten I have another visit in November at $750...do I have to pay the $750?

Deductibles are calendar year - yes. But in your example I'm assuming both charges occur in the same year?

Some carriers had something called a "carry forward" provision which meant if charges were incurred during the last quarter of the year then your deductible, it would count towards the following year's deductible. However, it seems most carriers got hammered on that and it's rarely seen.
