Why would Kane & Dupage have the fewest amount of Health insurance companies available to them.

Perhaps the doctors in those counties didn't want to take a pay cut? One of my senior clients couldn't get an appointment with any gynecologists in KANE county (population 520,000 as of 2011), because none of the gynecologists there accept Medicare.

Many of the on-exchange plans pay physicians and hospitals less than the off-exchange plans.

Dupage residents are wealthier than Kane's, so the reimbursement level might have been a big factor in that county as well, Sam.
