If he could lose 5-pounds he qualifies for 5-star graded. If he loses 20-pounds he qualifies for preferred.

The only diabetes question is Did you use insulin prior to age 40 or have you had complications from diabetes?

I just noticed the med for neuropathy. If that's related to his diabetes, that would be considered a complication. So graded only.

He should be able to get standard issue through Monumental. No height/weight restrictions and the depression meds shouldn't affect him because he is 56. Their question about that is before 50.

Foresters Level treats this pretty well. He does technically miss the Level plan by 3 lbs with their new Ht and Wt Chart, but thats just water weight ( just write him early in the morning:biggrin: )

$20,000 : $72.40
$25,000 : $89.71

Foresters rates are competitive, but with no height/weight restrictions, I'd opp for Monumental and get 1st benefit coverage in this particular case. Beware of possible phone interview if he didn't fully disclose all meds and something comes up on the pharmacy report.

Do you guys feel he will get Pr'd or Std with Monumental?

They have PHI's every case I have sent to them, few as they are. Wish I could just do POS.

Assurity might give you level. They don't care about insulin as long as no heart attacks/strokes.
The neuropathy might be an issue, they look back 12 months. I don't know if they are alright with maintence drugs.
