Excellent. Thank you for posting that.

The individual form is really a short-form for unmarried individuals who are not eligible for employer sponsored insurance and do not have other eligibility factors.

The long form is for families and those with other eligibility factors.

There is actually a 3rd application, which (strangely enough) is for people who want to buy their insurance on the exchange but don't want a subsidy. Apparently, you use these forms to apply to the exchange before you can pick a plan and apply for the plan. Yeah. Leave it to the government to make an extra step or two.

Note that the govt is excited that they pared it down from 21 pages. Actually, all they did was split the 21 pages into 3 different forms, and pare down a tiny bit. Then, they made appendixes on the long form. By doing it that way they can claim the 12-page long form is really just 7 pages, and they can claim the 5-page short form is really just 3 pages. Smart math, huh?
