Does anyone know if I can still submit the L120 life application? The girl I have that helps me gave me five of the L120s last week and I wrote two apps just checked the website and saw both forms available... thanks for your help.

Just think of the most difficult, ridiculous way that business could be done and you will have Monumental's answer. of course they will need an amendment no matter which way you go.:yes:
 
The new apps don't have the HIPPA form included. The only way I found them was to just print out the ones from the old app.

That is funny... when I contracted I was told no phi and then they did seven out of my first 8 with phi, So I started putting my number on the app instead of the clients so the interviewer calls me and I can then call the applicant.

I also started submitting medication list with applications and have had only 1 phi when submitting that way(didn't do this in the beginning).

I don't like the 22 page application and would rather just go ahead and require phi at the appointment. But I do like them.

I have never understood why companies don't include all required forms with the application. I'm not talking about the sometimes forms, but the ones that have to be with each and very application.

I can understand wanting it to be its own form for filing purposes, but include all required forms in the PDF or any paper app you send out. Make it easier on the agents.

AGLA has a 22 page application but it still doesn't inlcude all the forms.. There are more that you have to attach.

Pure stupidity. NGOs are as much the company's fault as the agents. Take out the sometimes stuff and put in the required stuff.

LFG has apps around that length. Missing tons of required forms, but includes the paperwork for additional insured. Gee, do I fill out the HIPAA or additional insured on every single application??
