I'm in a state where the client only has to answer the end stage renal question. Does anyone know if the client also has to answer the height/weight or smoker question?

With MOO yes they do.

A good rule of thumb is if the height/weight and tobacco questions are not a part of the health questions section they will have to answer them.

If they are included in the health questions section they will not have to answer them.
 
And the answer is, to my own question. When in doubt call underwriting. This for Illinois. Yes, list the height/weight, but they won't rate up, they have to put something in the system. No don't have answer smoker question.
 
In MI we don't have to answer height & weight but they do have to answer tobacco question. It's probably best if you call the underwriter and ask about your state

You are right, that should have been my answer, call underwriting.

The requirement for those two questions is state specific. In Indiana for example the agent always has to ask height/weight and tobacco question even during Open Enrollment and GI. In Illinois the height/weight and tobacco question when Open Enrollment and GI will not change the approval or rate but height/weight does need to be recorded. Next to the Tobacco question just put "Open Enrollment" or GI.

I've seen the same company, but in different states use different policies regarding ht/wt and tobacco questions during GI or OE applications.

I have one company that actually has the tobacco questions in with the health questions, but it has to be answered and will carry a rate no matter under what circumstances they are applying, GI, OE, etc..
