Notice of Open Enrollment

Anyone have/use or know where I can find some type of "Notice of Open Enrollment" form? I have a very large group that just did plan changes effective Sep 1st and I wanted to add something to the paychecks stating this is there open enrollment and the time to make final plan changes, add dependants etc. I will have to have it translated in Spanish due to the fact that 50% of the company speaks spanish only, I am really trying to CYA on this one since I do not.

Any advice is appreicated.
 
You should check with the carrier(s) on this - you DO NOT want to do your own translation. If a carrier has Spanish translation, great. Many do.
