I could be wrong, however I have yet to see a place where you can download such a document - at least - not a legal binding one. There might be examples out there, but I would recommend hiring a lawyer to draw up the forms.
All of our vested & producer agreements are looked over by our lawyer before they are signed. Someone out there might have a better solution for you, but I have yet to come across one.
