Tracking Form/Transmittal Form Help!

Does anyone have a tracking form or a Transmittal Form they could share with me...I used all of the one's that I had (I wrote on them all) and now American Amicable does not offer the form anymore. I used to use there's for all my carriers...but it's no more they said.

Example of the form, You know where the agent list the following:

Applicant's Name:
Age:
Soc-Sec #
Plan:
Modal Premium
Mode of Payment:
Cash with App.

I feel silly asking for this but I guess the folks never thought to keep the Original....shoot....

Make your own with a space for that info, your contact info and a space notes to the underwriter.

Make it fillable and it will be cleaner

I am so old school...I write all my stuff down so I can keep track of the numbers.

You mean I am not all alone in this old world? Thought I was the only one left that did things the "old way". After having a hard drive fry with no backup last year, I was kind of glad I did.

No you are not alone, I am always writing stuff down..after I had a computer crash I was like never again. Now I am looking for some Insurance software to track production and leads.
