Wells Fargo Transfer to annuity... where to send the transfer form?

Does anyone know? Know one at the bank or the 800# knows where transfer forms go to this megabank. She is doing a non qualified transfer to an annuity, she is using a CD to do a transfer out of Wells Fargo and cannot figure out where it goes.....??

Have the client do it at the bank and get a cashier's check payable to the annuity company, FBO your client's name.

Correction: Since it's non-qualified, just have the client move the CD money into their checking account and have them write a check to purchase the annuity.
