My wife has a friend who just got laid off and will lose medical benefits on the 10th of this month. Two adults and two children. One child is less than 3 months old and the other is a teen with Crohns disease.

I doubt they will be able to afford Cobra and I'm wondering if they will be able to afford private. What are the top issues I need to address with this family?

At one time my wife was told she had Crohns Disease. She was completely uninsurable until we got the incorrect information off her record.

Cobra and state pools will be the only option for that one.

Before you waste a lot of time...

I don't need their ages or state.

Find out whether they're gonna need the health insurance premium dollars to put food on the table, gas in the tank, and pay their rent/mortgage.
 
COBRA subsidy is 65% so likely they'll be able to afford it. If they can't afford to pay 35% of COBRA they don't stand a chance of being able to afford an individual policy.
 
Assuming the rest are healthy, put the Crohn's kid on COBRA. Look at options for the others.
 
The lady mentioned using a group plan under her business. I believe here in NC we can have a group of one. I have no idea how to do group. What options do I have to look into group for her?

Think about it like this. If they are not financially able to take COBRA (especially if it is reduced 65%) then they will not be able to afford a sole proprietor group plan where the risk pool is affected by Crohn's Disease. Their rate would probably be double of the standard COBRA rate.

There are two options:
COBRA for all (if they are eligible for the 65% subsidy)
COBRA for the Crohn's patient, individual plans for the rest.

Getting them off COBRA can be good in the long run in case they develop conditions that will make individual coverage impossible later. Then they would just be uninsured at the end of the COBRA period.
