Anybody use this paper form to apply for medicare?

Looks like it's just for part A. 40b form is used to pick up a/b

40b says this on instructions:

This form is your application for Medicare Part B (Medical Insurance). You can use this form to sign up for Part B: • During your Initial Enrollment Period (IEP) when you're first eligible for Medicare • During the General Enrollment Period (GEP) from January 1 through March 31 of each year • If you're eligible for a Special Enrollment Period (SEP).

Probably right. I've only had people apply for A and b online or go to their SS office. I've never given or looked for a paper option.
40b is for adding Part B.

The initial form posted by the OP would be for adding Part A. Usually someone that has to pay for their Part A. Not enough work credits and such.

I wouldn't use that one. Why open a can of worms when you don't have to.

Fax nothing. Ever. Online or they (the client) goes to the local office. Even going to the local office can be dicey.

i have to disagree with you on this at least with my local SS office experience.I have a current special part B enrollment which was done online and has been pending for more than 3 weeks.In the interim for another client i faxed the form l564 and 40b to local SS office it was processed within a week also just faxed form 40b for client on jan 2 enrolling in part B in the GEP and has already been processed.Some people just can't create the ID Me account and some of these same people won't travel to the SS office to do it it in person.

You can have my fax machine if you can pry it away from my cold dead hands.lol
