Yes, there are.
Pretty much state specific. Some states require that a company offering "any" medsupp plan to regular beneficiaries must also offer that same plan to underagers. The price is the trick. Gerber in MO for example makes it real clear that they don't want DI supplements. They price it at ~100% more than the T65'rs. Kansas requires that a plan offered must be also offered to underagers, but it has to be offered at the same price as T65's.
Asking general "who does what" questions won't get you very far. We see you're in TX, but is that the state you're asking about?
