Break Off Healthy from Group Plan

Look to see if the small group plan has a conversion policy. Convert the sick brother to a individual policy with no underwriting. It might work.
 
The other shoe drops (depending on your state DOI rules regarding group policies) when the healthy people drop off the group only leaving the unhealthy and their trail of claims. Have fun come renewal.

You also need to make sure this is not only their suggestion but their firm decision because I know in MD that any attempt to "blow up the group" - meaning peeling off the healthy is a DOI violation.
