I have an couple who's about 55 years old each & they have a 90 year old mother who's not licensed & doesn't drive. The mother just bought a car entirely in her name & my people want to insure it on their policy. They will use this car to drive their mother to/from doctor appointments etc. What do I do here?

They called the carrier directly (Safeco) who told them they can add the car & list the mother as an additional interested party? Doesn't seem totally right?

Depends on the company.

I have an elderly couple that is putting the home in the sons name and going to continue to live there. majority of companies consider this a dwelling fire but I have a company that will write a homeowners for the elderly couple and add the son as additional interest.

Same company allowed me to add a vehicle to a girls policy that was owned by her parents with them listed as additional interest

The suggestion from Safeco seems on-point with my experience writing business for a client who has a permanent disability and owns a car for others to transport her.

We looked at both ways of covering her and the client decided on getting own policy with the children as the drivers, but we also considered adding her car to one of the children's policy in this way.

sounds right to me. that's a good thing because the other option would be to add granny as an excluded driver. with some companies, that would cause a premium surcharge. I have no clue whether Safeco surcharges for excluded drivers or not.

but if Safeco is ok with just adding granny as an additional interest, I'd go with that.

Additional insured, not additional interest. It should be in the same drop down box...there is a difference in some states in case you don't have the option.

what's the difference in some states? they can't possibly be insuring a 90 yr old without a license that doesn't drive. Is this just a terminology difference from one company/state to the next?

In my state, interest only notifies the party if there is a change in the policy. i.e. Required to have insurance as a requirement on their job.

Additional insured protects the additional party. For example, an employee who has customers in their car. The employee's company may require the company be listed as an addition insured where the employees auto policy is primary. You see this a lot w/ Realtors.

As you can see, there is a huge difference.

Yes - Additional Insured

Even if she doesn't drive, she OWNS the car and is responsible for the use of the car. If there's an accident resulting in a lawsuit, she will be named in the suit - so she needs to be included as an insured on the policy. Read the Who Is An Insured section in the policy form. I bet it doesn't mention anything that would cover her short of the AI endorsement.
