Employer Offers but Does Not Contribute

Great answers. I am looking for the next step with a group plan that is over 9.5% of prospect's income. Prospect is ready to buy. I am concerned to do proper documentation, and make things work the first time through.

need from ER:
1. is prospect able to enroll in the next 90 days (question on exchange app)
2. get exact cost numbers from employer to fill out Appendix A form (see print version, Marketplace app), needed in case prospect is able to enroll in ER plan now (keep as documentation, if required to mail in to HC.gov or for IRS)

this is where I need help from forum:
3. how to answer online app questions in this case so prospect will be compliant and get passed through as eligible for subsidy.
