Are the Extra Help co-pays per prescription? Meaning if they get a 90 day supply, will they still only pay $4.90 for generics, regardless of how many days it's for or will they pay $4.90 per month, which would be 14.70/quarter?

Its per prescription, 30 or 90 day.

Per RX. Anyone that I've had on LIS who wasn't on an MAPD with $0 copays, I always advised to get 90-day RX when possible to save money.
