Do any of you use a form or folder to gather needed information about client? I worked for a very short time for a Med supo company and they had a "Needs Assessment" folder that asked questions about everything from age, children, all the types of insurance, investment and a place for referrals.
Is there such a generic form available or will it be best to make my own?
