My biggest concern: what am I responsible for?
At this point, I don't keep any PHI on my server/laptop/desktop (this includes copies of apps), I don't keep paper files, and any notes with health issues get shredded quarterly (and are in a locked box until picked up by the company who does that for me). I'm paranoid about the potential for a lawsuit, so I just don't have any information any more, even when it would make life easier.
