The Parkinson's client might not be out of the question. United Security Assurance might issue. Now, they have limited state approval, and they are not A rated, but they might. Also, look at an annuity (if he has the fund) that will increase income with ADL loss. It could be an alternative that can help. Also, is there a spouse? Very important to cover them. Mutual of Omaha has a rider that will give benefit to an uninsurable if the insured is also on claim. There are some outside the box alternatives out there.
 
Depending on how far he potentially is away from needing care, look at an annuity that will increase income with ADL loss, and insure the spouse (if there is one) Depending on the state, you can put the Security Benefit Rider on Mutual of Omaha that will give him a benefit if he has a claim while the spouse is also on claim. Neither of these options are the greatest, but it gives some sort of help to the household.

Most LTC benefits on annuity riders require the insured to have been able to perform all 6 of the ADLs when they first obtained the contract. So it could possibly work if they are still independent. But if not then it would not double the benefits.

True, but if there is current ADL loss, you can look at an impaired risk SPIA. All of these are taking large assumptions, that there is money to put away, etc. But the point is, there are options so it isn't a total waste of time.
