In the bank draft section on the app it asks for the name of the financial institution and their city/state. Then it asks for name, street adress etc. Is it asking for the bank's street address etc or the insureds?

Its asking for the name of the bank (and on the same line) the city & state where it was opened. Then the rest is the insure's info....address, ph #, etc for the bank acct (usually the same as the home address).
Great lol. I guess they dont really check that because Ive put the banks info there on some apps and the insureds on others.

You just got lucky. That is something they are very particular about. Also the routing number and account you write on there better match the voided check.
Make a mistake and they will require an amendment.
