Urgent Question

Form is typically specific to insurance company. In most states the dmv needs the original signature on the poa, just like the title. It may even have to be notarized.

Would that be something I could fax instead of mailing back?

I've had a rental for two weeks. They wanted me to return it yesterday... Which is why I'm rushing the payment.
 
Ah gotcha. I haven't worked claim-side but I wonder if it's possible to fight for more days as the idea of insurance is to make you whole again. Not sure though. There are limitations on rental reimbursement coverage (if you've paid a premium for it) when you are at-fault. But when you are not at-fault I figured they pay what they need to unless the at-fault party's limits have been reached and you'd be forced to sue the at-fault party for the extra few days.

I'm just trying to figure out who is requesting you return the vehicle. I understand that the at-fault party is paying for everything, I just wonder why you have to turn the rental back in if they are paying for it. If you are still waiting on their check, I would imagine there should be no reason for you to be returning the rental.
