VALIC Form Needed

Came across a VALIC Ira, In the past I have had to request forms to move valic money and they want to mail the form to the client. Does anyone know or have a Valic IRA outgoing rollover/ transfer form that is not specific or 'branded' for just that client.
Any help would be appreciated.
